JOB SUMMARY:
The Director of Care Integration is an innovative and experienced leader responsible for the operational, financial, programs and personnel activities for the assigned population health initiatives and/or departments. This leader works closely with the Admin Director to develop and establish long-term strategic plans for the department integrating the population health model of care, supported by data science that encompasses continuum of care coordination, social determinants of health, community collaboration and outreach. Works with a variety of stakeholders, coordinating the activities of population health across the organization. This includes implementing initiatives and continuously monitoring the goals and objectives for quality improvement of population health programs. Responsible for assuring compliance with all local, state, and federal regulations as well as accrediting organizations as they pertain to Population Health while maintaining alignment with the strategic plan for Harris Health System.

MINIMUM QUALIFICATIONS:

Education/Specialized Training/Licensure: Bachelor of Science degree in a relevant subject area such as Nursing, Public Health, Health Administration, Social Work, Business Administration or a related field. Master's degree in a related healthcare field preferred.

Certification: Basic Life Support from a hospital-based American Heart Association (AHA) approved program. Other certificates as required by unit/service.

Work Experience : 7 years' progressive, outcome oriented management experience of "population health" related programs such as health system management, health plans or public health in a large academic health system or integrated healthcare delivery system.

Management Experience : 5 years management experience with demonstrated success in planning, program development and operations with emphasis on sound business applications, effective quality/process management and efficient resource utilization. Excellent interpersonal skills to foster an interdisciplinary team approach to services and program delivery. Knowledge of DNV, State, Medicare/Medicaid and other external regulatory agencies.

Typical Duties That May Be Performed
1. Serves as an expert resource specific to services provided by the Diabetes Service Line. Provides strategic direction for the service line to ensure that appropriate services and programs are available to service line patients and their families. Annually reviews, evaluates, and adjusts the strategic plan.
2. Develops continuum of care programs based on identified needs by collaborating with administrators and physicians to assess, plan, develop, implement, and evaluate the service line programs for the Harris Health System.
3. Coordinates and facilitates a collaborative relationship with physicians and management to ensure appropriate program and facility growth, regulatory compliance and resource development.
4. Develops formal linkages with stakeholder health care providers in the system.
5. Establishes collaborative relationship with nursing and operational management to ensure that strategic objectives, customer satisfaction, and quality improvement practices are integrated in achieving desired outcomes.
6. Develops and implements a business and marketing strategy for the service line to assure community awareness and increased utilization of Harris Health System.
7. Responsible for developing and assisting in the preparation of grant proposals and overall development of budgets for strategic planning. Demonstrates sound financial management pertaining to service line initiatives.
8. Provides ongoing evaluation and assessment of customer needs and expectations. This evaluation and assessment is compared with present services to identify gaps and develop plans for improvement.
