Harris Health System seeks an experienced leader in patient care integration to join its department of Population Health Transformation, which has oversight of the system's transformative model of care encompassing the continuum of care coordination, social determinants of health, value-based financing strategies, community collaboration and outreach, and research and analytics. This clinical expert will collaborate in a fast-paced team environment to foster a comprehensive, cohesive, and coordinated approach to the care of patients over time and across a continuum of care settings, with a focus on improving outcomes and/or lowering costs for high-risk populations. The successful candidate will be able to demonstrate strong subject matter expertise; outstanding critical thinking, communication, and team management skills; experience collaborating with medical schools/clinical partners in a matrixed environment; familiarity with care compliance and accreditation standards; and a commitment to the diverse families we serve as the fourth largest safety-net health care system in the nation.

The Care Integration Manager is responsible for the operational, human resource, quality, and financial management of assigned area(s)/programs. This manager works with leadership to translate pavilion and department strategic goals into a plan suitable for the area's unique operational needs. The manager leads implementation and evaluation of the plan and provides reports relevant to achievement of goals. As applicable, the manager is responsible for assuring that Harris Health System care practice models are implemented and targeted outcomes achieved in assigned area(s)/program(s). The manager provides oversight to ensure care delivery, quality management, and staff utilization patterns are congruent with practice frameworks. The manager is responsible for fostering an environment that cultivates exemplary, integrated patient care. The manager mentors others, role models professionalism, and participates in professional development, research, and community activities.

MINIMUM QUALIFICATIONS:

1. Education/Licensure/Specialized Training:

a. Education:
i. Bachelors degree in Nursing or related field required (clinical background - RN, RD). Masters degree in health related field is preferred.

ii. If the manager is not responsible for supervising licensed nurses, a Bachelors degree in a related field is required/accepted.

2. Work Experience: Five (5) years of progressive, outcome oriented clinical experience with expertise in the care of high-risk patients.

3. Management/Experience: Two (2) years related management/leadership experience in program development and operations, with emphasis on effective quality/process improvement and efficient resource utilization. Excellent interpersonal skills to foster an interdisciplinary team approach to services and program delivery.
