Job description

JOB SUMMARY:

This position is responsible for the coordination of the transfer of patients from, to and between HCHD facilities. The Transfer Center Clinical Case Manager functions independently in the prioritization and verification of transfer requests. This position is also responsible for the maintenance of department records and statistics associated with transfer requests.

MINIMUM QUALIFICATIONS:

Education/specialized training/licensure:
Associate Degree or diploma required, BSN preferred. Current valid Registered Nurse License in the State of Texas. Basic Life Support Certification required.
Work experience:
5 years experience as a Registered Nurse. 1-2 years experience with Case Management, Quality Management, Coding or Discharge Planning preferred.
Management Experience
2 years management/supervisory experience preferred.
Equipment Operated
Must have Basic level computer application experience/skill (Excel, Word, etc.)
